Continuum runs on four habits. After a 1:1 or an incident you log what you noticed in two minutes, in your own words, and it lands on that person's running thread.
As you write, you tag the signals the entry carries — ownership, momentum, pressure — and which way each is pointing. Signals fade at a pace you set, so quiet counts as information too.
Those signals move your read of the person along in words, from uncertain to plausible to likely to certain. Every revision keeps its date and its reason instead of overwriting the last one.
Then Focus tells you where to look the moment you open the app: reads drawing fresh signal, people going quiet, anyone you haven't written about since the reorg.
